Prenatal Personal Training forms
Because we care about you and your pregnancy, we require a completed and signed (by you and your healthcare provider) PARmed-X for Pregnancy form from all our valued pregnant clients. Be sure to bring a copy to your first personal training sesion as we do not make exceptions to this policy.
In addition to the PARmed-X for Pregnancy, please download our prenatal personal training intake forms. One of these forms must be completed and sent back via email at least 48 hours before your first training session.
